The slide sequence

What makes this deck fail
- No non-goals slide. Scope creep starts with things nobody explicitly excluded.
- Owners listed as teams. 'Marketing' does not attend meetings or reply to messages; a person does.
- A task-level Gantt in the kickoff. Nobody reads it, and it is wrong within a fortnight.
- Risks with no owner. An unowned risk is an observation.
- Leaving without recorded decisions. The kickoff's output is agreement, and unrecorded agreement evaporates.
